Ms. Wainwright:

Are you requesting agency care for a Program admitted claimant?  If so= , what agency will be providing the care for the admitted claimant?  W= hat skill level of care will be provided for the 20 hours per week that you= are requesting?  Where is the doctor's letter of medical necessity to support the request?  Will these 20 hours rep= lace other hours of care the admitted claimant was previously receiving, li= ke family caregiving hours, for example?  Is this a temporary, or long= -term request?  Please explain.

The parent or guardian of the admitted claimant is supposed to request agen= cy care, not a representative of the agency.  It poses a potential con= flict of interest.  Additionally, if a parent wants the Program to com= municate directly with an agency representative, the parent will need to provide the Program with a Release to permit that.=

I am attaching forms you may need to have completed if agency care is being= requested.  Any agency who wants to provide care for an admitted clai= mant must provide the Program with a copy of its employment policies concer= ning criminal history and sex offender registry background checks, Certifications that each employee who will car= e for an admitted claimant has not been convicted of a barrier crime, a new= reimbursement rate agreement, and an authorization.

Please note that the Program does not afford respite care for parents. = ; I'm happy to answer any questions or concerns you may have.

I am Katrina Wainwright from Hope In Home Care, requesting this service at = 20 hours weekly for his father to return to work.
I requested it through the Acentra portal for he has Virginia Medicaid also= . They asked if Va Birth Injury Fund was covering it fully or partially? I = am requesting either authorization of coverage from BIF or denial of covera= ge.
