Hi I wanted to provide an update on the stat= us of this order. As mentioned previously, the provider's clinic has been h= aving severe fax issues, delaying the order process. There are several docu= ments in need of the providers signature before we are able to arrange for = the delivery of Mr.Barbour's wheelchair. The providers clinic has informed = me the patient has an appointment January 15, 2026 at 4PM EST regarding the= chair. I have re-sent the required documents to the clinic per their reque= st and have attached a copy here as well in the event you may be able to as= sist us in getting these documents signed and returned. Please let us know = if you have any questions or concerns. Thank you !
